Prayer Leader Contract

Please indicate the date of the prayer service that you plan to lead
Thank you for sharing yourself and your gifts with the JCM community. By agreeing to participate as a prayer leader, you agree that you are responsible for the end-to-end start and completion of your service. Please be ready for the service 15 minutes beforehand. Your requested items will be ready for you when you arrive unless requested earlier. JCM will not reimburse you for anything not expressly approved by the Executive Director and the Rabbi. We are willing to help set up and clean up as agreed ahead of time-please put in your JCM requests above everything you will need from us.  A JCM staff or volunteer will be here to let you into the building unless otherwise decided.
